Before you begin

Please read these expectations carefully before completing the benevolence request form.

1. Please call our church office at 429-5541 when you have completed all areas of the form. Office hours are Tuesday through Friday, 8:30 a.m. to 4:30 p.m.; the office is closed on Mondays. After hearing back from you, a deacon will contact you by phone.
2. This form must be completed before your deacon conference and brought with you. If you do not complete and bring it, your conference will be cancelled.
3. We are not a rescue organization. If your need is immediate or an emergency, please seek help elsewhere.
4. We consider helping those willing to make hard but necessary changes in their lifestyle to improve their situation and who are making positive steps in that direction.
5. We welcome the opportunity to discuss your situation and pray with you. Remember that your help comes from the Lord.
